For those who have the courage to read it, and in memory of 14-year-old Dennis Lindberg and the many others like him. This source tells it just the way it is: Boy dies after refusing blood

Blood Transfusion: Letter of Understanding
by defender of truth inhttp://ajwrb.org/science/blood-transfusion-letter-of-understanding.
"every parent among jehovahs witnesses worried about how local watchtower appointed elders will respond to letting their child have blood transfusion without opposing it should download this letter of understanding and be ready to hand a copy to them.
then ask those elders to leave them (the parents) and doctors alone to concentrate on the childs best interests.".

Do you know if this document is used anywhere besides Canada?
I don't have copies from any sources other than from Canada, but the document is in widespread use inside and outside Canada based on what is published in medical literature on the subject. For instance, Nationwide Children's Hospital in Columbus, Ohio has developed a training seminar for clinicians that heavily admonishes use of the document in pediatric JW cases.
Also, in the 2005 incident of the 14-year-old Canadian JW who left Canada for treatment at Schneider Children's Hospital in New York the agreement reached between the US medical facility and the JW girl's family (with Watchtower's support!!!) was the same as found in the Letter of Understanding document. My opinion is that the horribly bad press Watchtower was suffering at the time in cases like this compelled it to relax its 1992 position, which allowed for this agreement that was later framed as a form document for institutional use. But to the day Watchtower has not once advertised this agreement (or the form) among JWs for use by parents and their children. This lack is one reason for my blog article on the subject. Advertise! Pregnant women refuses blood
by Giordano inhttp://jehovahswitnessreport.com/blog/jw-refusal-of-blood-transfusion-mother-and-baby-die#more-4001.
a 28-year-old woman and her unborn child have died because of the womans decision not to accept a lifesaving blood transfusion.. doctors at the royal hospital for women and prince of wales hospital in randwick, sydney, australia, have described the harrowing effect on hospital staff of two otherwise avoidable deaths.. the woman, a jehovahs witness, was seven months into her pregnancy when it was discovered she had leukaemia.. amy corderoy, health editor, reporting in the sydney morning herald states,.
more than 80% of pregnant women suffering from the cancer, called acute promyelocytic leukaemia, will go into remission with proper treatment, and the outlook for their babies is good.. .

Any references for this? I've no recollection of such a teaching.
Here:*** w69 4/1 p. 224 Questions From Readers ***
Later on Watchtower published more on this subject, but stopped short of saying the 1969 information was wrong. Instead Watchtower advised not to speculate on way or another.
Applying that to miscarriages and stillborn deliveries, is it reasonable that in the future Jehovah will insert back into the womb of a woman a partially developed embryo, or possibly a number of them? No, that does not seem so, nor is it likely that women who have had this sad experience actually expect that.
Furthermore, resurrection is for persons who have lived as individuals before Jehovah. Even a child who lives for only a short time after birth has existed as a separate person. But a miscarried fetus or stillborn child, though from a Biblical standpoint considered a “soul” while it was developing, never actually lived as a separate and distinct individual. So it would appear that such situations do not fall under the resurrection provision outlined in the Bible.—Acts 24:15.
We fully appreciate that this view might be most disappointing to some. -

...maybe they were concerned you were somehow going to contest the document and make it more difficult for them to protect children by causing legal issues.
I was told the reason for plying me about why I wanted the form document was because of the institution was "still sorting out our processes regarding freedom of information (FOI)." That excuse struck me as pure poppycock. But I was finally provided with the form so I didn't make further inquiry about whether someone with a Watchtower affiliation was involved in the process of handling my request. I just dropped it.

I remember when I began researching this material. It took years to finally get a copy of the Letter of Understanding. It was a very frustrating experience.
When I finally found a healthcare facility that had one of these documents known to be approved by Watchtower for use by JWs, I was sent through the proverbial ringer with questions about why I wanted the form document. I finally asked the institutional contact whether she had affiliation with Watchtower because of hesitancy to share a document that should be freely available to anyone who asks. I finally received the form document with a lame explanation about why I was plied with questions about my request. I never received an answer to whether the institutional staff member had affiliation with Watchtower. But I finally did receive the document, which I was thankful for.

Among Watchtower's more diabolical deceptions is how it presents information to children for their consumption. It teaches JW children to believe something that is demonstrably misleading about healthcare choices and even gives these children a printed cheat-sheet on the exact lie they should tell. See Watchtower’s Version of “Truth”

Watchtower's misrepresentation of CT Russell on 1914 gets more explicit: Taught what about 1914?
Every organization on the face planet earth has lots of error buried in its archives, but to lie about the error and extent of the error is telling, and to keep on lying about it is again telling.

This is the kind of publicity the Watchtower organization hoped to avoid with its move to relax its blood taboo in year 2000. But in this case Watchtower's change didn't help because this JW refused anything and everything extracted from blood that might have otherwise helped save her life and that of her unborn fetus. According to the published article in Internal Medicine Journal, "The patient was a Jehovah’s Witness with an advanced care directive refusing the transfusion of red cells, white cells, platelets or plasma, and all minor fractions including albumin, immunoglobulins and clotting factors."
I've been in the room more than a few times when JWs died over this god-damned teaching. Watchtower's Governing Body members, and all those who support this particular doctrinal position are guilty of crimes against humanity insofar as I'm concerned. I know what they're doing, and so do they.

The mathematical probability of spontaneous order (no designer/creator)
by Fernando inthere seem to be many prerequisites for life as we know it.. to name a few: order, function, compatibility, availability, sustainability, intelligence, consciousness, intuition and so on.. focusing on only one, namely order.. what are the chances of order arising spontaneously, by chance, with no creator/designer?.
i have often pondered this and recently came across a mathematical summary of the big picture:.
if every particle in the known physical universe (10^80 particles), participated in one trillion interactions (10^12 interactions) per second, for the entire 30 billion years of the universe's existence (10^18 seconds), then we would by now have covered only 10^110 permutations.. if you had only 100 components in a container, what are the chances that a blindfolded person could lay them out in order on a table?.

What are the chances of order arising spontaneously, by chance, with no creator/designer?
What are the chances of someone in particular winning the multi-million dollar lottery? Astronomically small.
What are the chances that someone will win the multi-million dollar lottery? 100 percent.
